When to Use This Form? (यह फॉर्म कब काम आता है?)

This certificate is a valid supporting document for:

• New Aadhaar Enrolment: For new applicants who lack standard POI/POA documents.
• Aadhaar Update Request: For existing Aadhaar holders who want to update:
  • Name (नाम)
  • Address (पता)
  • Date of Birth (DOB) (जन्म तिथि)
  • Mobile Number / Email (Note: Address proof is often needed for mobile updates at centers)
• It is valid as Proof of Identity (POI), Proof of Address (POA), Proof of Relationship (POR), and Date of Birth (DOB) document, as per UIDAI regulations.

Who can Certify this Form? (फॉर्म को कौन प्रमाणित कर सकता है?)

The form must be filled and signed by an authorized certifier. The certifier's details (Name, Designation, Address, Contact) must be clearly mentioned. The authorized certifiers include:

• Gazetted Officer - Group A
• Gazetted Officer - Group B
• Village Panchayat Head or Mukhiya
• MP / MLA / MLC / Muncipal Councilor
• Tehsildar
• Head of Recognized Educational Institution
• Superintendent / Warden / Matron / Head of Institution of Recognized shelter homes or Orphanages
• EPFO Officer

How to Fill the Aadhar Update Form (फॉर्म भरने के निर्देश)

To ensure your form is accepted, follow these instructions carefully:

1. Print Quality: Print the downloaded PDF on a clean A4 size paper. It is not required to be on a letterhead.
2. Block Letters: All details must be filled in BLOCK LETTERS only.
3. No Overwriting: Overwriting on the form is not allowed. If you make a mistake, print a new form.
4. Resident's Details: Fill your Aadhaar Number (for updates), Full Name, C/o (Care of), complete address, and Date of Birth.
5. Photograph: Paste your recent color photograph (size 3.5cm x 4.5cm).
6. Certifier's Action: The certifier must sign and stamp the form. They must also cross-sign the photograph (signature partly on the photo and partly on the paper) and put their stamp.
7. Validity: This certificate is valid for 3 months from the date of issue. You must submit it to an Aadhaar center within this period.
